Faxes Accepted Pending Receipt of Originals
The FDNY Pension Bureau announced it will be accepting walk-in and faxed WTC Notice of Participation Forms at HQ until midnight tomorrow (June 14). NOTE: Mailed notices that are postmarked by that date, but received after June 14 2007 will not be accepted.
A Pension Bureau employee will be available to accept your form until the deadline at midnight. All forms must be notarized prior to filing since the Dept will not have a notary available. Members who show up at HQ in person to file their forms after 6 pm tomorrow (June 14) should have the security guards call Randy Shaw, ext. 7041 or Mary Basso at ext. 1306 for access to the 6th floor.
If you fax your form (fax to 718-999-0107), it must be signed and notarized before faxing it and you MUST also mail or hand deliver your original form (return receipt is always recommended) after you fax it. The Pension Bureau advises you to put on the front of the envelope being mailed that it was faxed in with the date and time!
