IRS 1095 Forms to be Mailed to ALL Members

Once again, due to IRS regulations, All Active and Retired members will receive a 1095B form from the UFA/SBF, in addition to similar forms you will receive from your NYC health-plan. The UFA has begun mailing this form to your home address. This form reports your covered dependents to the IRS and is mandatory as part of the affordable care act (ACA). Members can file their income taxes before receiving the 1095 form, as long as you notify your Tax Preparer that both you and your dependents have healthcare coverage.

Please review the information included in the 1095 form when you receive it. Check to make sure the names, dates of birth and social security numbers for yourself, your spouse and all of your dependents are correct.

The UFA continues to request updated information on your dependents (date of birth, social security, etc.). If you have not sent in this information or if the information on the 1095 from is incorrect, please call the SBF at 212-683-4723 and press 2 for a Benefits Associate. You can also email
