UFA GROUP LIFE INSURANCE RESTRICTIONS - RETIREES/PROMOTEES
The following benefit upgrade restriction is reprinted from the Group Term Life Booklet (also delegate manual W-15, pg.6); The life insurance benefit amount to be carried into retirement will be based upon the amount of active insurance you had in effect one year prior to retirement date. Must have had an upgrade approval before the one year cut off date. Later upgrades will be excluded from retirement formula. The UFA subsidizes retirees premiums with the following exception: Effective March 1999, UFA will no longer offset premiums of new promotees above the rank of Firefighter/Fire Marshal/Wiper. The total premium must be paid by post 3/1/99 promotees wishing to maintain the UFA Group Life Insurance Policy.

ON-DUTY APPARATUS ACCIDENTS - CHANGES AT DMV
Please photocopy this 65-2 bulletin and place this section in the appropriate location in the company office and/or bulletin board. It is imperative that members in the field are brought up to speed on the recent changes which took place at the Department of Motor Vehicles (DMV). We thank the chiefs over at Safety for informing the union, for the good of both parties.

  1. The correct FDNY insurance code is 994 (not the old 0035).
  2. That 994 code must be written in black/blue ink (not red like before).
  3. You must give your address “as it appears on your NYS Driver’s License”. You can no longer use a mailing address or FDNY location.
  4. As per Safety Bulletin #56, Section 24.1.3, the large “F” to be placed in the DMV Use box on the MV-104 shall now be in RED ink.

* NOTE) If the address given does not coincide with that which is registered under your motor vehicle operator number, it is now DMV policy to automatically SUSPEND your license for failure to report. Filled out correctly, according to these new rules, there remains the proper protection for all drivers.

CRIME RELATED INCIDENTS - CFR/D. FROM DELEGATE MANUAL B-14, PG.5.
We have seen many teleprinter tickets that relate to suspected crimes in progress. Pay special attention to section 3.14 of AUC 312, as it outlines the use of “extreme caution and restraint in their arrival at the incident”. This is not being done on a regular basis. Also, “companies should request police assistance from the fire dispatcher and confirm police arrival at a scene before intervening to begin medical care”. Comply with these policy guidelines religiously. You “should NOT attempt to enter a scene ... before the arrival of police”. If necessary, remind your officer of these directions. Do not jeopardize your safety due to an UNSAFE ACT or UNSAFE ORDER. Have the FDNY dispatcher monitor a

ONE REASON YOU DO NOT OPEN SUSPICIOUS PACKAGES!
On August 7, 2000 a suspected improvised explosive device was located outside a residence in Savannah, Tennessee. The recovered device components consisted of a plastic tool box, a photoelectric switch, a six volt battery, wiring and two twelve inch long PVC pipe bombs. The tool box was wrapped in brown paper and addressed to the owner of the residence. The photoelectric switch may have been used to initiate the device upon opening the tool box. The remains of the suspected device are in AFT custody and will be submitted to the ATF lab for analysis. Extreme caution should be exercised when conducting an investigation involving suspected devices in closed containers.

ARE YOU THINKING ABOUT FIREFIGHTER/BRAVEST LICENSE PLATES?
Due to changes that have taken place at the N.Y.S. Department of Motor Vehicles, if you desire to purchase a Firefighter/Bravest license plate, you no longer call DMV directly. You must now contact the UFA office for an application and that completed application will be mailed back to the UFA office. Ask to speak to Debbie Lynch at extension 5948.

Do NOT call DMV directly any longer. Revised applications will be included in an upcoming Delegate Manual update package.

CIGNA Retirement & Investment Services, your retirement plan provider, has made improvements to its Defined Contribution record keeping system, which will now allow you to make transactions via AnswerLine or AnswerNet until 4:00 PM Eastern Time, each business day.

Currently you must execute any transaction through AnswerLine or AnswerNet by 3:00 PM Eastern Time on any business day, in order to receive the net unit value for that day. Effective August 20, 2000, you will have until 4:00 PM Eastern Time to execute transactions and still receive that day’s net unit value.
